Medistar Home Health Now Part of UnitedHealthcare’s Care Provider Network

BATON ROUGE, June 3, 2013— Louisiana residents enrolled in UnitedHealthcare’s employer-sponsored, MedicareDirect and Evercare/SecureHorizons Medicare Advantage health plans now have access to care provided by Medistar Home Health.

The new network relationship with UnitedHealthcare allows a greater number of residents the benefit of receiving high-quality of home health care from one of the nation’s top health care organizations.

Medistar Home Health Selected as Humana Home Health Network Preferred Provider

BATON ROUGE, LA--(Marketwire - Nov 28, 2012) - Medistar Home Health today announced that is has been selected by Humana in its first competitive bid process in the state of Louisiana. Such process covered four of Humana's Louisiana regions and Medistar was chosen to provide home health services throughout the state, including the regions in Humana's bid process, Baton Rouge and Shreveport, as well as some areas in its New Orleans and North Shore regions. In making this selection, Humana reviewed and considered Medistar's scope of services, quality of care, financial metrics, and geographic service area.

A current partner and provider of care to Humana's members throughout Louisiana, Medistar has been chosen as one of the select few home health agencies to continue serving these Louisiana regions and this will allow its members the benefit of receiving the highest quality of care possible from one of the nation's top home health care organizations.

Medistar Home Health Chosen as First Post-Acute Care Provider in Louisiana's Health Information Exchange

Medistar's Successful Use of Electronic Medical Records (EMR) is Applauded as They Join the

Baton Rouge, LA (PRWEB) May 29, 2012 -- Medistar Home Health announced today that it has joined the Louisiana Health Information Exchange (LAHIE) as its first post-acute care provider. LaHIE is the state’s exclusive mechanism that allows for the secure exchange of health information among authorized providers and across Louisiana’s health care system to help improve patient safety, quality of care and health outcomes.

Hospitals and physicians must achieve “Meaningful Use” of EMR in order to comply with the HITECH (Health Information Technology for Economic and Clinical Health) provisions of the American Recovery and Reinvestment Act of 2009. Meaningful Use Stage 2 criteria focuses on how healthcare facilities and physicians can securely exchange key clinical information about patients. LaHIE and its participants are well ahead of many other states in the nation in establishing a comprehensive secure exchange network, which will also eventually include pharmacies, labs and other post-acute care providers.

Medistar Home Health Achieves Low Readmission Rates Using Electronic Medical Records and Predictive Modeling Tools

Post-acute care provider Medistar Home Health shows significantly lower readmission rates
than national average.

Baton Rouge, LA (PRWEB) May 22, 2012 -- Hospitals with readmission rates deemed excessive by Medicare for heart failure, heart attack, and pneumonia will see their Medicare reimbursements reduced starting October 1, 2012. In an effort to reduce 30-Day rehospitalization rates, many hospitals are turning to post-acute care providers for help in monitoring patients’ health and compliance with discharge instructions after patients leave
the hospital.

According to a newly released report, post-acute care provider, Medistar Home Health, has readmission rates far lower than the national average of hospitals as well as that of home health agencies. The hospital readmission rate for heart attack patients is reported at 19.8%, heart failure at 24.8% and pneumonia at 18.4%.Medistar Home Health’s reported averages were significantly lower for each of these diagnoses. The reporting organization, OCS Homecare, a division of Outcome Concept Systems (OCS), is a national leader in performance improvement reporting and analytics for home health and hospice organizations.
Integrated into Medistar’s EMR system, the OCS Predictive Modeling program assigns patients a risk level for hospitalization and emergent care, which aids in customizing and adjusting care plans as patients’ health condition change while under home health care service. Medistar Home Health attributes low readmission rates to efficient practices, use of EMR and the overall quality of patient care its staff provides patients.

Medistar Home Health Sponsors Christmas Food Drive

Medistar is proud to give back to the community through an internal Christmas toy drive.

Medistar Home Health, a home health provider in Louisiana, is proud to have participated in The Providence House’s Christmas food drive for needy families in the Shreveport and Bossier area.

The Providence House is a Shreveport based organization that has been helping fight poverty for the past 18 years. Its goal is to provide families with a hand up, rather than a hand out. They offer programs that educate families on how to break the cycle of homelessness through skill building and responsibility.

“This has been a rough year for a lot of families in the Shreveport and Bossier area” said Beth Denton CEO and owner of Medistar Home Health, “as a home health agency our employees naturally want to take care of the people around us. Our caring staff felt that helping families and children have a better Christmas was something we had to do.”

Medistar’s Shreveport-Bossier City office collected turkey, ham, canned goods, household items and Christmas cards to distribute to needy families in the area.
